Dear Survey Participant: We are seeking participation in a graduate level survey research project at West Virginia University School of Medicine, Division of Occupational Therapy. This project serves to provide us with a better understanding of teacher's perceptions of school-based occupational therapy. By understanding how teachers perceive occupational therapy, we can perhaps find ways to collaborate in order to better serve the students in the future. We are asking all teachers on this listserve to complete our online survey at the website link below. All responses will automatically be emailed to the researchers upon completion, with the website listed as the return address.Before responding to the questionnaire, you should read the information on the research website which explains your rights as a potential research participant.Should you elect to respond, please do not place any personally identifying information on the questionnaire in order to insure confidentiality.Please recognize that your decision to respond or not respond will in no way impact your membership in this listserve.Thank you for taking the time to read this introductory letter and we hope that you will elect to complete the online survey.
