It sounds as if you may be dealing with more than one problem.

The difficulty getting information onto paper is called dysgraphia. There is an email support group at http://groups.yahoo.com called dysgraphia that you might want to join. Parents there can be very helpful with information.

The problems with motor skills sound like vestibular problems (rhythm, timing, balance) and sensory integration. An occupational therapist can evaluate these issues. (An OT can also do some of the diagnostic testing for dysgraphia.) You usually get a much more thorough evaluation through a private OT. Find the best one in your community and then give their clinic a call. They deal with medical insurance all the time and may be able to tell you exactly how a referral needs to be worded or coded by your physician to ensure coverage by medical insurance.
