You need to get The Peterson's guide to colleges for students with Learning Disabilities or the K&W Guide.
They both list the admissions requirments for LD students. I believe most schools expect LD students to meet the same requirments as regular applicants. I know the university of Vermont allows you to petition to the admissions comitte, if they don't accept you the first time you apply.
