Quality of Services questionnaire

Hello,
I am looking for parents who are willing to answer a questionnaire about the quality of services in special education their child(ren) is/are receiving from public education.
The survey is for a graduate school class I am taking on collaboration. Please, I would appreciate your help.Please email me at JuliaKLarson@gmail.com so I can email you a copy of the questionnaire.
