ACT Testing accom/ Generalized Anxiety Disorder

Our daughter is entering her junior year in high school, and we are preparing to request accommodations for the ACT. She has been diagnosed with dyslexia, ADD without hyperactivity, and significant Generalized Anxiety Disorder. (all well documented back to elementary school) Ideally, she’d be granted 50% extra time, and permission to take the test over two days with extra breaks to address the anxiety issue.

Does anybody have any experience in documenting anxiety issues with the ACT Board, or have any specific suggestions about who should send documentation (psychologist/therapist, psychiatrist, psychologist that did psycho-educational testing, school learning specialist, etc.? Also, what specific type of documentation for anxiety issues is needed? Thanks!!
