Does anyone know where I could download a copy of the evaluation form for ADHD? It’s the one you give to your child’s teacher. It’s just a one page questionair. They answer questions regarding your child’s behavior in class and at school. I can probably get one from the doctor but I was hoping to get a copy in a hurry. Anyone have an idea for me? I know I have seen the form online before but I can’t seem to remember where the website is. Thanks for any help.

I am new to this. (See my recent post. I have not figured out how to start a new post/thread. Just resond). Is this evaluation form one to get feedback on a daily or weekly basis? If so, could I also get a copy and how do I do that?
